Post-Incident Evaluations

After an incident wraps up, evaluations help your team capture what happened and what could be improved. They’re structured questionnaires that your organisation configures — so every debrief follows the same format.
Fill out an evaluation
Section titled “Fill out an evaluation”- Open the incident and go to the Evaluations tab
- If you’ve been assigned an evaluation (or if evaluations are open to all), you’ll see the questionnaire
- Answer the questions and add any additional notes
- Click Submit
Questions can be:
- Text — free-form written answers
- Number — numeric values
- Yes / No — simple toggle
- Single choice — pick one option from a list
- Multiple choice — select all that apply
Some questions may be marked as required.
View submitted evaluations
Section titled “View submitted evaluations”Everyone on the incident team can see submitted evaluations. Each one shows who submitted it, when, and their answers — useful for comparing perspectives across the team.
Set up evaluation questions (Editors)
Section titled “Set up evaluation questions (Editors)”Editors configure the question templates in Settings > Evaluation Questions:
- Add questions with a type (text, number, yes/no, single choice, multiple choice)
- Mark questions as required or optional
- Target specific groups — if your organisation has user groups set up, you can show different questions to different roles (e.g. one set for operations leads, another for field crews)
- Reorder questions by dragging
- Archive old questions without losing past answers
- Evaluations work well for both real incidents and training exercises — use them to review practice runs too
- Keep questionnaires short (5-10 questions) so people actually fill them out in the field
- Use a mix of structured questions (yes/no, choices) and one open text field for anything the structured questions didn’t cover
